Medical treatment of Intestinal perforation with Typhoid fever


Abstract
A case of typhoidal perforation of the intestine with succesful medical conservative treatment is i presented and clinical aspects are briefly reviewed.
Twenty-seven year old man was admitted with complaints of fever, vomiting, abdominal pain with distension and general malaise which were persistent for 20 days.
Physical examination showed marked debilitated condilion and abdominal distension with tenderness. Laboratory examination of blood revealed leucopenia (4100/mm3) and Widal reactions showed 1:160 for Salmonella typhi OH and paratyphi A.
Radiologically, large amount of subdiaphregmatic free air revealed bilaterally in Chest P-A view and Plain abdomen.
The patient was markedly improved with conservative medical treatment comprising of fluid and electrolyte, several antibiotics, abdominal decompression by gastric suction, frequent enema and abdominal hot compress.
